One issue I have seen is people that want to do the "speed reholster" to get more reps in whatever drill they are doing, or just think that it looks good.  I would think that if reholstering were a slower and more deliberate act, the chances of leaving your finger in the trigger guard or the thumb break getting into the trigger guard would be lessened.

My limited experience shows that it happens mostly when someone is distracted - ie thinking of something ELSE other than reholstering the weapon in a safe manner.

On the bright side...at least he didn't muzzle sweep his body with an issued submachine gun...that could be really ugly.
