Great topic regarding clearing guns. At the range on Saturday, one of the guys was instructing a "new to the range shooter" to remove the magazine BEFORE racking the slide to clear the weapon. He explained why and then he told the story about how he didn't remove the magazine once and ended up putting a round through his wife's bedroom mirror.
Regarding the IDPA, both clubs at which I shoot, give slow steady instruction on clearing the weapon and the SO looks down the pipe also. With my HK USP, I have a decocker.
I was never in the AO, but my friends say that the unloading barrels were installed because there were so many accidents. As in many cases, issues learned with the US Military should be used in the civilian world. I like Ben's drill and the point he made about being precise about it. When this action is SO important and it becomes old hat, is when AD's occur.
I used to catch my rounds when unloading until one day I twisted too much in the process not exactly being unsafe, but it felt like I could be unsafe. My rounds hit the deck now.
